This is done automatically and it works well together with other modules such as PathAuto, Global Redirect, Path Redirect, Revisioning, Smart menus, Smart tabb, as well as the core Block and Statistics modules.

Installation:

Unpack in your modules folder (usually '/sites/all/modules/')
Edit 'inlcudes/path.inc' and add "return path_alias_xt_get_path_alias($path, $path_language);" under line 109 OR
Put a compiled PECL runkit extension file into the appropriate /extension directory, edit the path_alias_xt file and uncomment the "//dl('runkit.so')" file
Put the website temporarily offline
Enable under Administer > Site Building > Modules
